Streaming timeout defaults

Maximum idle time between provider stream events. Without an explicit override, the default is 150 seconds, or 300 seconds for effective named reasoning effort high, xhigh, or max (including model defaults and runtime changes). Numeric budgets and on/off reasoning retain the base default. Explicit values always win; None disables idle timeout enforcement. Provider-specific service tiers may have longer automatic defaults.
